  2. Department
  3. Contact information (email, phone)
  4. Location of Incident
  5. Hardware affected (Computer, projector, etc.)
  6. Application or Function where problem is occurring
  7. Error Message (if any)
  8. Description of the problem. As appropriate, provide specific details on the problem. For example, Type of file, folder affected and/or workstation or printer ID.
